On Sept 23, 2008 the National Biodefense Science Board (NBSB) held an afternoon meeting to review the report and recommendations for the National Disaster Medical System (NDMS) that had been prepared by the Disaster Medicine Working Group. The Working Group had assembled an assessment panel which evaluated previous reports done on the NDMS and put together their own report and specific recommendations which, with the approval of the NBSB, would be transmitted to the Assistant Secretary for Preparedness and Response.
Kevin Yeskey, Director of the Office of Preparedness and Response and Deputy Assistant Secretary for Preparedness and Response began the meeting by giving an overview of NDMS and its capabilities. He also highlighted the recent response to hurricanes Gustav and Ike and contrasted NDMS participation to what occurred in response to hurricane Katrina. For Gustav and Ike they had been able to mobilize fully equipped teams, totaling 1100 response personnel rather than the 37 in the wake of Katrina.
Next, Dr. Stephen Cantrill, the Chair of the Working Group presented the assessment panel’s key recommendations and the NBSB had the chance to go over each and comment. Below is an outline of the recommendations, as the panel chose to organize them.
Recommendation 1: Envisioning the Future
1.1 Develop a clear strategic plan for the NDMS and integration of all other disaster medicine response mechanisms.
1.2 Develop a civilian advisory group for NDMS.
Recommendation 2: Integrating the Past
2.1 Establish a formal mechanism to track the implementation of lessons learned and recommendations from after-action reports.
Recommendation 3: Strengthen the team
3.1 Ensure teams are fully staffed and well-equipped
3.2 Improve and streamline application process for all personnel – *This point was removed during the discussion because based on Director Yeskey’s opening presentation, the NBSB determined that this has been done. Yeskey reported that the application and hiring process has been reduced from 8-12 months to 42 days.
3.3 Implement a uniform training program across NDMS.
3.4 Put into place a system to register non-overlapping personnel so that response capability can be precisely accounted.
Recommendation 4: Serving the patient
4.1 Determine who the patient is, consider patients that have been displaced across state lines NDMS patients even if they were not receiving NDMS care.
4.2 Expand reimbursement so that it is not limited to NDMS hospitals. Ensure that it is timely.
4.3 Define a concept of operations for patient moving and tracking.
4.4 Expand Electronic Medical Records for field use.
4.5 Examine barriers to patient care in emergency response and define criteria for temporary suspension of HIPPA or other requirements during an emergency.
Recommendation 5: Engaging Partners
5.1 Partner with state and local organizations.
5.2 Seek out public/private sector partnerships for patient transport and care.
Recommendation 6: Secure Funding
6.1 Funding is inadequate for the program. Seek out new sources for sustained funding.
Recommendation 7: Future
7.1 Request feedback from ASPR for the NBSB Spring/Summer 2009 meeting on which of the recommendations are complete, in progress or will not be undertaken.
7.2 Recommend that a longer term follow-up study be conducted to focus on the NDMS and these recommendations – * This point was added by the NBSB during the discussion.

The next NBSB meeting will take place Nov 18-19, 2008 and the Board is expecting to hear a presentation from the Disaster Mental Health Subcommittee, a report from the Medical Countermeasure Processes Working Group, a report from the Personal Preparedness Working Group and receive a white paper from the Education and Training Working Group.

The new nuclear policy paper National Security and Nuclear Weapons in the 21st Century published quietly Tuesday by the Defense and Energy Departments embraces the “lead and hedge” strategy of the first Clinton administration for how US nuclear forces and policy should evolve in the future.

Yet the “leading” is hard to find in the new paper, which seems focused on hedging.

Instead of offering different alternative options for US nuclear policy, the paper comes across as a Cold War-like threat-based analysis that draws a line in the sand against congressional calls for significant changes to US nuclear policy.

CNN and AFP are reporting that the Shabaab, a militant wing of a Somali insurgent group, the Islamic Courts Union (ICU), has threatened to treat “as an enemy combatant” any plane that attempts to land at Mogadishu Airport.  According to AFP, the threat, which was posted on the Internet, was confirmed by Shabaab leader Mukhtar Robow.  The web posting reportedly includes a list of grievances used to justify the threat, including the airport’s use by “Ugandan and Bulgarian mercenaries,” money generated by the airport for the Ethiopian government, and harassment of “Somali religious personalities” by “US and Israeli secret services…”  The warnings are accompanied by a graphic of a man pointing a shoulder-fired missile at a plane as it is landing.

The threat is not to be taken lightly. Last year, the FAS identified Somalia as one of three MANPADS proliferation hotspots worldwide in response to numerous reports of illicit missile activity, most of which involved the ICU and the Shabaab.  In 2006, UN investigators identified at least six shipments of MANPADS and other weapons to the violent Insurgent group, including a shipment of “50 units” of ”shoulder-fired surface-to-air missiles and second generation infrared-guided anti-tank weapons” from Eritrea, “45 units” of surface-to-air missiles from Iran, and three surface-to-air missiles from Syria.  In each case, the missiles were part of larger arms shipments that also included dozens of assault rifles, machine guns, and other small arms and light weapons.  The Associated Press later reported that the ICU had received 200 shoulder-fired missiles from Eritrea alone.

In March 2007, the Islamists fired two advanced SA-18 missiles at a Belarussian cargo aircraft as it was departing from Mogadishu International Airport.  One of the missiles hit the plane, causing it to crash and killing all eleven people on board.  UN investigators later concluded that the missiles used in the attack were part of a consignment of six SA-18s acquired from Eritrea.  This summer, the UN traced another SA-18 found in Somalia back to a batch of Russian missiles that were shipped to Eritrea in 1995. The Eritrean government denies allegations that it provides missiles and other weapons to the ICU.

At a House Committee on Foreign Affairs meeting 9 September 08 members of Congress discussed what the U.S. response should be to Russia’s aggressive actions in Georgia last month.  Most members of the Committee acknowledged that some sort of response was necessary to voice U.S. concerns about the possibility of a more aggressive Russian foreign policy, while they also conceded that the U.S. does need Russia’s cooperation on important matters of international security, such as helping to influence Iran’s nuclear ambitions.  An interesting debate is being hosted online on this topic by The Economist – click here to link to it.

Description: FAS is looking for a smart, energetic individual to help direct a major web-based project using innovative video, multimedia and social networking tools to revolutionize the way biosecurity issues are communicated. The initial work will focus on building and development of an interactive and easy to use web portal. Unique content will be created and updated daily. This project is part of an ambitious FAS program to use web 2.0 principles and new technologies to provide comprehensive information on biological and chemical weapons. Sarkozy says he wants to be more open about France’s nuclear arsenal “than anyone ever has been” about theirs. OK, does the aircraft carrier Charles de Gaulle still carry nuclear weapons in peacetime?

The French nuclear weapons arsenal currently includes approximately 300 warheads, according to our latest estimate published in the Bulletin of the Atomic Scientists. Roughly 80 percent of the warheads are for delivery by three ballistic missiles submarines. The remaining warheads are on cruise missiles for delivery by land- and sea-based strike aircraft. The total arsenal is expected to decrease further to some 290 warheads in the next few years.

Although President Nicolas Sarkozy declared in March 2008 that he had “decided that France could and should be more transparent with respect to its nuclear arsenal than anyone ever has been,” France remains the only European nuclear weapons state that has not yet declared whether its aircraft carrier still carries nuclear weapons under normal circumstances.

In an earlier blog post, arguments were discussed from a 12 June 08 meeting of the House Committee on Foreign Affairs for and against the signing of a civilian nuclear cooperation agreement (123 Agreement) with Russia. At the time, the most salient issues were our ability to influence Russia’s position vis-à-vis Iran’s nuclear ambitions and the possibility that the 123 Agreement would restart domestic reprocessing, reversing 30 years of US policy. Since then, a full scale military operation has taken place between Russia and Georgia, a newly democratic ally of the U.S. who sent 2,000 troops to support U.S. efforts in Iraq. Now both Russian and American leaders want to remove the 123 Agreement from consideration for the time being, so as not to allow current events to color any debates about passing the legislation. Those in favor of the 123 Agreement believe that it would open up greater cooperation with Russia on issues such as pressuring Iran on its nuclear program. Whether this is true or not, if the 123 Agreement is now off the table because of Russia’s actions in Georgia, how much has this conflict damaged our ability to cooperate with Russia on nuclear arms control in the future?
