SAFETY AWARDS

AFI 36-2833, 13 May 1994, is supplemented as follows and applies to all 21st Space Wing units and personnel, to include geographically separated units (GSU). This supplement does not apply to tenant units receiving host support, US Air Force Reserves or the Air National Guard. Section W--Air Force Space Command Individual Ground Safety Award of Distinction - CATEGORY I/ QUARTERLY; CATEGORIES II, III, AND IV/ANNUALLY

89.6 (Added). Nomination Procedures. Nominations for Category I must reach 21 SW/SE no later than the 13th of the month following the fiscal year award period (Jan, Apr, July, Oct). Nominations for Category II, III, and IV must reach 21 SW/SE no later than 1 November.

Section X--Air Force Space Command Unit Safety Award - ANNUAL

95.5 (Added). Nomination Procedures. Nominations for Category I and all qualified units in Category II and III must reach 21 SW/SE no later than 1 Nov in order to reach HQ AFSPC/SE by 15 November.

Section Y--Air Force Space Command Crew Safety Award of Distinction - QUARTERLY (AFSPC Sup 1)

101.5 (Added). Nomination Procedures. Nominations for Categories I through VI must reach 21 SW/SE no later than the 13th of the month following the end of the quarter award period (based on fiscal year).

Section Z--Air Force Space Command Safety Award of Distinction - QUARTERLY (AFSPC Sup 1)

107.6 (Added). Nomination Procedures. Nominations for Categories I through VI must reach 21 SW/SE no later than the 13th of the month.

Section BB--TEAM 21 UNIT GROUND SAFETY AWARD - ANNUAL (Added)

116. Purpose of the Award. Team 21 Unit Ground Safety Awards are presented to 21st Space Wing units in recognition of outstanding achievements toward an effective unit safety program and for significant contributions toward the 21st Space Wing's Mishap Prevention Program.

117. Who is Eligible. Every unit assigned under the 21st Space Wing. Units will be considered in the following categories:

117.1. Category I. All 21st Space Wing units located within the PAFB complex (PAFB, CMAS, FAFB)

117.2. Category II. All 21st Space Wing GSUs.

118. Selection Criteria. Selections are based on a unit's contribution to the Mishap Prevention Program and the unit having no reportable mishaps during the fiscal year. Areas of consideration include, but are not limited to:

118.1. Mishap Rates. Reportable on and off duty mishap rates, first aid injury trend rates, and AF motor vehicle accident rates.

118.2. Safety Meetings. Attendance at Bi-Monthly Unit Safety Representative and Combined Wing Safety Council meetings. Contributions to mishap prevention and hazard abatement programs.

118.3. Education and Publicity. Attendance, by unit members, at Supervisor Safety Training and Motorcycle Safety Foundation Training courses. Unit personnel involvement in the safety program.

118.4. Inspections. Management involvement in the mishap prevention program. Timely responses and correction of program and facility discrepancies.

119. Nomination Procedures. Anyone assigned to the 21st Space Wing may submit an award nomination. Nominations must reach 21 SW/SE no later than 1 Nov following the fiscal year award period. Prepare nomination letters in one original (no more than two single-spaced typewritten pages). Include:

119.1. Unit, Name, Rank, and Duty Phone of submitter.

119.2. Category of Award.

119.3. Organization and number of personnel assigned.

119.4. A brief summary of the unit's safety accomplishments and contributions to the Mishap Prevention Program.

120. Selection Procedures. Wing Safety personnel will review and submit nominations to the Ground Safety Manager (GSM) no later than 7 Nov following the fiscal year award period.

120.1. The GSM will review all nominations for accuracy and completeness, and submit recommendations to the Chief of Safety for final selection.

120.2. The Chief of Safety will make the final determination and approve the two category finalists for the Annual Unit Ground Safety Awards.

121. Awards Ceremony. The awards will be sent to the Wing Commander for presentation at an appropriate ceremony.

121.1. Each year a recognition plaque will be presented to the Unit Commanders.

121.2. The Safety office will announce the winners in the Safety Net newsletter and the Space Observer newspaper.

Section CC--TEAM 21 INDIVIDUAL GROUND SAFETY AWARD - QUARTERLY (Added)

122. Purpose of the Award. Quarterly Team 21 Individual Ground Safety Awards are presented to individuals in recognition of a specific act, outstanding achievement, or significant contribution to the 21st Space Wing Mishap Prevention Program.

123. Who is Eligible. All personnel, military and civilian, assigned to the 21st Space Wing to include the GSUs.

124. Selection Criteria. Selections are based on an individual's contribution to the safety program and will be determined by the Chief of Safety based on some, or all, of the following factors:

125. Quarterly Team 21 Individual Ground Safety Award.

125.1. Establishment of a special ongoing or one-time project enhancing safety.

125.2. An action that prevented or reduced loss of life, injury, property damage, or mission degradation.

125.3. Outstanding contribution to safety observed by a member of wing safety staff.

126. Nomination Procedures. Anyone assigned to the 21st Space Wing may submit an award nomination. Nominations must reach 21 SW/SE no later than the 7th of the month following the fiscal award period (Jan, Apr, July, Oct) Prepare nomination letters (one original) no more than one single-spaced typewritten page and include:

126.1. Name, Rank, Unit and Duty Phone of submitter.

126.2. Category of Award.

126.3. A brief summary of the individual's specific act, accomplishments, or contributions to the 21st Space Wing's Mishap Prevention Program.

127. Selection Procedures. Wing Safety personnel will review and/or submit nominations to the Ground Safety Manager (GSM) no later than the 10th of the month following the fiscal year award period.

127.1. The GSM will review all nominations for accuracy and completeness and submit recommendations to the Chief of Safety no later than the 15th of the month following the fiscal year award period.

127.2. The Chief of Safety will make the final determination and approve the finalist for the Quarterly Ground Safety Award.

128. Awards Ceremony. The award will be sent to the Wing Commander for presentation at an appropriate ceremony.

128.1. Each quarter an award will be presented to individual winners.

128.2. The Safety office will announce the winner in the Safety Net newsletter and the Space Observer newspaper.
